Patients who have a long term condition, as above, will recieve only one letter to ask you to attend for your annual review.

We are working towards your review date being your Birthday, so it will be easier for you to know when your annual review is due.

Whilst we are changing your review date if you know you are due for a review or are poorly controlled or having medical problems just book an appointment with our Practice Nurse.
